MAYFAIR EYE CARE: SHARE THE CARE PROGRAM DETAILS

How: It’s easy! All you have to do is let your friends and family know how much you enjoy your vision care at Mayfair Eye Care and once they book with us, we will take care of the rest. At the end of each calendar year, the referrals will be tallied and our patients will be notified of their reward. 

Who: This program is for current patients of Mayfair Eye Care who refer other people including family and friends for eye care to our office.  For those who are not our patients, and refer people to Mayfair Eye Care, we have our ‘Personal Touch’ program. Contact our office to learn about the Personal Touch program.

A referral is defined as any individual who books and attends an appointment with an optometrist at our clinic.

All patients are automatically enrolled but no patients are required nor in any way obligated to refer other individuals to our clinic. The automatic inclusion is simply a way for us to keep track of any referrals that you provide to us so we can genuinely say thank you to you. If you wish to opt out of this, please advise us and we will be happy to remove you from this program.

Timing: The current ‘calendar year’ runs from now until December 31.  At the end of the calendar year, any outstanding reward levels reached that have not received their reward gift, will be issued their referral gifts.

Once you have earned a referral level such as “Level 1: Two new patient referrals”, you will either be notified by the clinic that your reward is ready for pick up or you may receive certain rewards in the mail. Our clinic aims to tally the patient referrals every 3 months and issue rewards at that time.

Rewards: Reward gifts are not exchangeable or redeemable for any other value, item or discount. Gifts shall be accepted as awarded. Mayfair Eye Care reserves the right to issue rewards and does not owe any monetary value to patients who refer others to the clinic.

Referral process: New patients will be asked how they heard about Mayfair Eye Care and who referred them to the clinic. If they state your name as a referral source, your patient file will be noted with credit for this referral. If you believe there has been an error or you have any concerns, please contact our office at any time to discuss it.

Program changes: Mayfair Eye Care reserves the right to change or update this program as deemed necessary by Mayfair Eye Care. The program may also be terminated at any time at the sole discretion of the clinic.
